Title: Reentry Care Coordinator

$30.00 - $31.00 / Posted Thru: 4-30-24

30 Hours Per Week

Location: Remote/Virtual - Must Live in Michigan

Purpose: Under the supervision of the Program Manager/Supervisor, the Reentry Coordinator will be responsible for implementing individual youth reentry plans and coordinating case management activities to assist juvenile offenders' transition back to into the community. The Reentry Coordinator attends and presents at the Juvenile Justice Program Specific Transfer Training (JJPSTT), state assisted training programs.

Duties and Responsibilities:

Using a wrap-around model, develop appropriate and effective reentry plans that meet the needs of each individual youth. Components of the plans include but are not limited to housing, transportation, mental and physical health services, medications, substance abuse services, employment, education, family engagement, community supports, and other needs as identified in the individual youth plan.

  • Housing Providers- Establish relationships with rental housing providers within each community that the target population returns to. Ensure that youth for whom returning to their family's home is not an option have a stable housing alternative.
  • Community Mental Health Providers- Facilitate referrals to community mental health providers to ensure that youth returning home from residential placement have access to effective, individualized mental health treatment services.
  • Social Service Agencies- Connect returning youth and their families with social service agencies that provide access to state and federal entitlements that support the youth/family during the transition from residential placement.
  • Education Systems- Collaborate with the Michigan Department of Education and local school districts to develop processes for re-enrolling youth in their home schools post-release and ensure that academic credits obtained while in residential placement are transferred to the school they attend in the community.
  • For youth who are more likely to obtain a GED than return to the traditional secondary school setting, provide referrals to GED prep courses in the youth's community. When appropriate, assist youth with exploring post-secondary education opportunities.
  • Workforce Development Agencies- Collaborate with local Michigan Works! agencies to equip returning youth with the skills to find and maintain employment that allow them to be self-sufficient. When possible, build relationships with community businesses to facilitate the creation of employment opportunities for returning youth. Work with agencies to prepare youth for employment opportunities.
  • Substance Abuse Providers- Facilitate referrals to substance abuse providers to ensure that youth returning home from residential placement has access to effective, individualized substance abuse services.

Other Duties.

  • Screen and assess each eligible youth six months prior to planned release using the Child and Adolescent Functional Assessment Scale (CAFAS) and /or daily Living Activities - 20 tools to determine their individual needs for pre and post release program supports.
  • Engage the youth and their families in the reentry service plans and planning process to obtain maximum family participation in visitation, treatment, reentry team meetings, and other services necessary for reintegration into the community.
  • Develop collaborative relationships within each community where youth are released. Builds relationships with and ensures strong alliances in the community with agencies and organizations.
  • Coordinate the use of entitlements and state resources, when possible, to support youth's reentry plan.
  • Attend and present the Juvenile Justice Program Specific Transfer Training (JJPSTT).
  • Collect baseline data metrics.
  • Conduct research to understand industry and employment trends and skills needed by job seekers.
  • Develop marketing materials in conjunction with MDHHS that create awareness of the reentry service program and describe referral eligibility and process.
  • Maintain accurate statistical records and prepare reports.
  • Prepare written and oral reports as well as related documents as needed.
  • Maintains a positive, strong, credible, professional, and interpersonal relationship with all parties relevant of MPHI projects and represent to the best interest of MPHI at all times.

Qualifications/Requirements

Education: A bachelor's degree in Sociology, Psychology, Counseling, or related subject area preferred. Teaching experience is highly desirable.

The ability to speak Spanish is highly desirable.

Experience: Two years ofpolicy analysis/development experience required. Knowledge of Medicaid and behavioral health systems strongly preferred.Ability to demonstrate understanding and awareness of the employment related concerns of offenders transitioning into the workforce. Ability to develop and maintain a strong relationship with other community organizations, knowledge of career counseling and job search techniques. Knowledge of community resources available; knowledge of Case Management principles, local labor market information and support services available required.

Work Environment and Physical Requirements:Will require in-state travel. Requires a valid vehicle operator's license and access to a vehicle to attend off-site meetings, conferences, and trainings.
