As a full time, exempt CPS Solutions, LLC Physical Therapistfor SturgisHospital'sRehabilitation Department, you will evaluate and develop rehabilitation goals to increase self-care and instrumental activities of daily living for inpatients and outpatients with identified mental, developmental, or physical disabilities. Duties include evaluations; communication with patients, family members, and rehabilitation medical team members on a collaborative plan of care; and establishment of goals to enhance independence with activities of daily living in a safe, effective and reliable manner. Knowledge of computer-based documentation is a must.

Schedule:This full time position is scheduled to work 40 hours a week.

About the Facility: Sturgis Hospital is a 84-bed licensed, acute care facility. Nearly 40 active medical staff members, available within the Sturgis and surrounding communities, represent a wide variety of specialties including Anesthesiology, Family Practice, General Surgery, Gynecology, Internal Medicine, Obstetrics, Ophthalmology, Orthopaedics, Pathology, Pediatrics, Podiatry, Radiology, Urology and Vascular Surgery. Additional specialties are offered through the Hospital's Courtesy, Consulting and Affiliate Medical Staff. These services include Cardiology, Neurology, Neurosurgery, Oncology, and Pulmonary medicine.Sturgis, Michigan is centrally located between Chicago, IL and Toledo, OH (both roughly 2 hours), and between Lake Michigan and Lake Erie.CPS Solutions, LLC has partnered with the hospital through our division, CPS Solutions, LLCRehabilitation Services, to manage the day-to-day operations of this department. This position is an employee of CPS Solutions, LLC.

About the Rehab Department: Hours of operation are Monday through Thursday from 8:00am-6:30pm,Friday from 8:00am-4:30pm, and weekends on a patient need basis.

Required Qualifications:

  • Bachelors' Degree required; Master's degree preferred
  • Michigan Physical Therapist license in good standing
  • One year of acute care and outpatient experience preferred

Hospital Requirements:

  • (PPD) TB Skin Test - Proof ofnegativeTB skin test within thelast 12 months
  • (MMR) Measles, Mumps and Rubella or A Blood Titer proving immunity
  • Varicella -(2)documented doses or A Blood Titer proving immunity
  • (Flu) Influenza required for hire between Oct 1st-April 30th
  • HepB Titer or declination
  • TDaP vaccine
  • COVID Vaccination (required)
